Some of us remember special times with extended family gathered at a long table or around the house. When I was a kid there was a predictable pattern to Thanksgiving. We grew up living next to my mother's parents who were part of my everyday life. The cooking would begin early and both houses would be full of the smells of onions, sage, thyme, rosemary and all encapsulated in the warm steam created by all the cooking. It was always an exhausting day, filled with both the joys and the craziness that typifies family.<br />
<br />
Now when I say craziness, I think everyone knows what I mean. Families have patterns and dynamics that interweave all of our interactions. Surely some of these dynamics play out more positively or more negatively in different family systems. As of these memories tend to come back, unless they were traumatic, we usually remember the more positive ones.<br />
<br />
Among the siblings birth order has a lot to do with the interplay between parents, grandparents, and children. A friend of mine clipped a page out of a catalog recently on which there were three T-shirts you could purchase. The first T-shirt was printed with 'I am the oldest , I make the rules". The second T-shirt said "I am the youngest the rules don't apply to me" . The third T-shirt, the one most appropriate for me personally said " I'm the middle I'm the reason we had rules " !<br />

arrived on the South Shore.
FINALLY! I know that some of us
thought it would never come. But the passing of the summer
solstice means that we will soon have conditions that will require us to think
carefully about our clients as well as ourselves. <o:p></o:p></span></div>

<span style="font-size: large;">The two most common issues
that we need to be aware of are adequate hydration and overheating. As you
know, many elders feel cold even on the warmest days. As a result they often
will have the windows closed on the hottest and most humid days of the summer.
They feel comfortable, but of course it can be potentially dangerous. It is
also important in those situations that you, the Caregiver, take care of yourself as well. </span></div>

<span style="font-size: large;"><br /></span></div>
<div class="MsoNormal">
<span style="font-size: large;">What
are the signs of overheating? They are profuse sweating, fatigue, and muscle
cramps. More advanced symptoms can include headache, dizziness, weakness,
nausea, vomiting and dark urine. This could be the beginning of heatstroke
which is a much more dangerous event. These symptoms include fever, confusion,
rapid shallow breathing and even unconsciousness. </span></div>

<span style="font-size: large;">What is first aid for
something like this? Have the person lie down in a cool place. Raise the
clients feet about 12 inches. Apply cool, wet cloths to the person’s skin and
use a fan to lower body temperature. If they are showing more significant
symptoms as noted above call 911.<o:p></o:p></span></div>

<div class="MsoNormal">
<span style="font-size: large;">Dehydration is the real
danger in hot weather. Some of these symptoms are similar to heat exhaustion.
They include increased thirst, a dry or swollen tongue, headache and confusion.
The best way to avoid dehydration is prevention. Make sure that your client and
you are consuming enough fluids. Consider sports drinks which contain
electrolytes and decrease the likelihood of developing dehydration. Remember
our clients especially if they have dementia may not realize they are thirsty.
So you may want to encourage them and remind them to drink. Obviously keep in
mind any restrictions they may have.<o:p></o:p></span></div>

millions of Americans. It impacts many
of our clients as well. Those suffering
with it now are using a number of medications, most of which have been in use
for several years. There is a need for
new medical treatments to manage and hopefully someday eliminate this disease. The process of developing these new medications
can take years of research and clinical trials.

clinical trials for investigational medications to treat Alzheimer’s disease in
association with Donald S. Marks, MD, a Board Certified Neurologist, specialist
in Alzheimer’s disease and an Assistant Clinical Professor at Tufts. <o:p></o:p></span></div>

the community at large. Over the many years that we've been providing services
to residents on the South Shore we have become acutely aware of the critical
need for good information about resources, management of certain types of
problems such as dementia care and help for families who are struggling with
the sometimes extreme stress of caring for a family member. </span></div>

that makes Visiting Angels different, is our deep professional roots in human
services. We have worked in inpatient hospital settings, transitional care
units, nursing homes, outpatient practices, and provided teaching to graduate
and undergraduate students and at National and International conferences. Because
our Co-founders and Director are all licensed clinical professionals with collectively over 100 years of clinical, rehabilitation, social work, and
teaching we've brought our expertise to as wide an audience in the community as
possible. </span></div>

The study ranks the top five home care companies where male and female caregivers want to work, compiled by surveying thousands of professional caregivers. This particular edition of the study also highlights key retention factors and highlights the differences between what’s important in the workplace to men versus women. </div>

“Job stability and learning opportunities were reported as the most important retention factors for males while job stability and the hours/shifts available were the biggest driver for women,” said Brandi Kurtyka, Chief Executive Officer of Chicago-based myCNAjobs, in a statement. </div>

About 40% of respondents were under 30 years of age, while another 42% were age 31-50 and 18% were 50 or older. Seven in 10 reported wanting to work 36 or more hours a week, and if they don’t get the hours they want, nearly 40% plan to get multiple caregiving jobs, while 38% reported plans to seek work at a hospital or senior care center that would offer the hours they want. </div>

Mary is a woman who has few visitors and whom I’ve volunteered to spend a little time with every week. I greeted her, complimented her on her beautiful turquoise sweater, and shook her hand. </span><br style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;" /><br style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;" /><span style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;">Then I sat down at her little table that was overflowing with books, photographs, the newspaper and other items she wants to keep close at hand. I started off by picking up a small framed photo of Mary with her husband and three children – two sons and a daughter. </span></div>

That’s a tip I picked up from The Best Friends Approach to Alzheimer’s Care by Virginia Bell and David Troxell.</span><br style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;" /><br style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;" /><span style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;">“Oh, her name is Connie,” she told me. “She has four children – two boys and two girls.”</span><br style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;" /><br style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;" /><span style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;">She continued by giving me several details about Connie and her family. I then picked up a photograph of Mary and her twin sister, Bernice, and she told me about how they took piano lessons together when they were children. After a few minutes I asked her if her daughter ever played a musical instrument.</span><br style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;" /><br style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;" /><span style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;">“I don’t have a daughter,” she said matter of factly.</span><br style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;" /><br style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;" /><span style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;">“Oh,” I countered, picking up the family photo again and holding it out for her to see. “You just told me you have a daughter. Here she is.”</span><br style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;" /><br style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;" /><span style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;">Mary’s face fell and she said very quietly, “I guess I do have a daughter.”</span><br style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;" /><br style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;" /><span style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;">I immediately felt sorry for her embarrassment and was disgusted with myself for having pointed out her mistake. I realized I’d just broken one of the cardinal rules for interacting with a person who has dementia. I’d just read it in The Best Friend’s Approach that very morning: “Let the person save face.”</span><br style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;" /><br style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;" /><span style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;">When relating to a person with Alzheimer’s there are many guidelines to follow. I’m going to discuss five of the most basic ones here: 1) Don’t tell them they are wrong about something, 2) Don’t argue with them, 3) Don’t ask if they remember something, 4) Don’t remind them that their spouse, parent or other loved one is dead, and 5) Don’t bring up topics that may upset them.</span></div>

There’s no good reason to do that. If they’re alert enough, they’ll realize they made a mistake and feel bad about it. Even if they don’t understand their error, correcting them may embarrass or be otherwise unpleasant for them. </span><br style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;" /><br style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;" /><b style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;">Don’t Argue With the Person:</b><span style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;"> It’s never a good idea to argue with a person who has dementia. First of all, you can’t win. And second, it will probably upset them or even make them angry. I learned a long time ago, when caring for my beloved Romanian soul mate, Ed, the best thing to do is simply change the subject – preferably to something pleasant that will immediately catch their attention. That way they’ll likely forget all about the disagreement. </span><br style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;" /><br style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;" /><b style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;">Don’t Ask if They Remember Something:</b><span style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;"> When talking with a person who has Alzheimer’s it’s so tempting to ask them if they remember some person or event. “What did you have for lunch?” “What did you do this morning?” “Do you remember that we had candy bars when I visited last week?” “This is David. Do you remember him?” Of course they don’t remember. Otherwise they wouldn’t have a diagnosis of dementia. It could embarrass or frustrate them if they don’t remember. It’s better to say, “I remember that we had candy the last time I was here. It was delicious.”</span><br style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;" /><br style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;" /><b style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;">Don’t Remind the Person that a Loved One Is Dead:</b><span style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;"> It’s not uncommon for people with dementia to believe their deceased spouse, parent or other loved one is still alive. They may be confused or feel hurt that the person doesn’t come to visit. If you inform them that the person is dead, they might not believe it and become angry with you. If they do believe you they’ll probably be very upset by the news. What’s more, they’re likely to soon forget what you said and go back to believing their loved one is still alive. An exception to this guideline is if they ask you if the person is gone. Then it’s wise to give them an honest answer, even if they will soon forget it, and then go on to some other topic.</span><br style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;" /><br style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;" /><b style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;">Don’t Bring up Other Topics That May Upset Them:</b><span style="background-color: #fefdfa; color: #333333; font-family: 'Trebuchet MS', Trebuchet, sans-serif; line-height: 18px;"> There’s no reason to bring up topics you know may upset your loved one. If you don’t see eye-to eye on politics, for example, don’t even bring it up. It may just kindle an argument, which goes again the second guideline above. You won’t prevail and it’s just likely to cause them anger and/or frustration.</span></div>

the holidays great! <br />
<br />
<b>1.</b> <b>Involve your loved one in the preparation.</b> You can give your
loved one small tasks such as signing holiday cards, hanging ornaments on the
tree, assisting with holiday baking, and cooking, sorting decorations, flower
arranging, table setting, and household chores, such as folding laundry, and
dusting. <br />
<br />
Plan their involvement during family gatherings. <br />
<br />
Peeling vegetables, stirring ingredients, folding napkins, helping set the
table, passing out gifts, can all benefit you and your loved ones holiday
experience. <br />
<br />
Leaving a loved one with Alzheimer’s sitting alone or without appropriate
activity and inclusion can evoke distress, sadness, or behavior problems.</span><a href="http://www.blogger.com/null" name="more"></a><br /><span style="font-family: "Trebuchet MS", sans-serif; font-size: large;">
<br />
<b>2.</b> <b>Make sure you have all medications and supplies filled and ready
for the holiday season.</b> <br />
<br />
There are medication management pharmacies that can simplify obtaining and
dispensing medications. They deliver, coordinate multiple physicians’
orders, verify orders monthly, and prepackage individual dosages, including
over the counter medicines and supplements. <br />
<br />
Also have a full supply of any incontinence products, or medical supplies
available, so you do not have to get supplies at the last minute.<br />
<br />
<b>3.</b> <b>Evaluate the environment where you will spend the holidays.</b> If
you spend the holidays at home and your loved one lives with you, they are
familiar with their surroundings and have a bedroom to rest in or retreat to,
when they need relief from activity. <br />
<br />
If you’re loved one is visiting your home or another family member’s home you
will need a bit of preparation. <br />
<br />
Make sure the environment is barrier free, and safe. Remove throw rugs, and
reduce clutter, for persons with balance or ambulation problems. Keep a change
of clothes available for any mishap with spills or incontinence. If there are
special diet needs, including the texture of the food or drink, make sure the
host or hostess is aware, or plan to bring your loved ones meal and/or drinks.<br />
<br />
<b>4. Prepare a quiet area in</b> <b>advance.</b> Excessive stimulation can
lead to anxiety, irritability, and exhaustion. <br />
<br />
Plan for quiet rest, and a nap. Frequent breaks from the chaos of a house full
of people can prevent anxiety, and exhaustion. If your loved one starts to
appear anxious, take them to the quiet area before the anxiety escalates.
Monitor facial expressions throughout the day as they often mirror underlying
emotion. Fidgeting, hand wringing, picking at clothing or skin, pacing, and
general restlessness are common signs of agitation. <br />
<br />
Providing soft music and familiar items in the quiet area, as well as a soft
and gentle massage can reduce the anxiety. <br />
<br />
If your loved one is on medication for anxiety, you may want to pre-medicate
before the holiday activities begin -- if the doctor’s order is as needed. If
there is an anti-anxiety medication ordered and it hasn’t been given prior to
agitation, make sure you give the medication at the first sign of anxiety, as
it could take up to an hour to be effective.<br />
<br />
<b>5. Think about holiday traditions in your loved ones younger years and
incorporate those into the holiday season.</b> Alzheimer’s patients are able to
recall long term memory much easier than short term memory. Was there special
music, food, decorations or activities that were enjoyed years ago? Are there
photo albums or stories from holidays past? This can often be an activity to be
shared between your loved one and younger family members. <br />
<br />
Photos and stories often provoke a memorable conversation of how grandma or
grandpa enjoyed the holidays years ago. <br />
<br />
The activity of sharing with the younger family members creates new memories. Are
there old radio programs or music that can be found on UTube to initiate
conversation and memories? Did your loved one enjoy music and singing? You can
initiate a sing-a-long of their favorite songs of the past.<br />
<br />
Many Alzheimer’s patients who can’t carry on a conversation retain the ability
to sing or read. Are they able to read Christmas stories to the younger ones? A
quiet break in the day with a Christmas story read to the family can be a much
cherished activity.<br />
<br />
<b>6. Reduce embarrassment.</b> Alzheimer’s patients are vulnerable to
embarrassment, especially in the earlier stages of the disease. <br />
<br />
If incontinence is a problem, be discreet, and don’t announce planned toileting
or change of incontinence products. Educate the family prior to the events not
to say things like <i>don’t you remember</i>, or <i>I just told you that</i>. <br />
<br />
The fact is each time an Alzheimer’s patient repeats themselves, they believe
they have only said it one time. So family members need to respond as if it’s
the first time. Redirection can work wonders, so redirect your loved one to
another area of the home, or to an inclusive activity. <br />
<br />
If you find yourself getting impatient, ask another family member to take over
involvement while you take a break, or ask someone to involve your loved one in
a simple activity. You may need a quiet break as much as your spouse or parent.<br />
<br />
Do not put a “bib” on your loved one during meal time. Either change their
clothing prior to the meal, or plan on changing after. “Bibbing” an elderly
person can initiate comments from younger family members in particular, like
“why does Grandma where a bib?” We do not know the specifics of what an
Alzheimer’s patient can or cannot understand. <br />
<br />
It is extremely important to provide dignified care no matter how advanced the
disease is. Do not belittle your family member with comments or actions that
would make them feel like a child or present them as such. They are adults with
a disease, they are not children.<br />
<br />
<b>7. Plan for help.</b> Additional help can relieve the caregiving responsibilities
with the holiday frenzy. If you have family members to provide respite ask for
help, and schedule it. Have a family member provide care while you shop, attend
to preparations, or just rest. If a family member is unavailable, private duty
homecare can help wonders.<br />
<br />
Caregiver’s can attend family functions with your spouse or parent so you can
be relieved of the caregiving duties and just enjoy family. They can also bring
your loved one to and from a family function for a specific time period. As an
example, if you know the holiday events will be too chaotic or lengthy, you can
have a caregiver bring your loved one to only the dinner portion of the event,
and return home with them until the event is over and you can resume care.<br />
<br />
Caregivers can also help your parent prepare for the holidays by assisting with
sending cards, baking, shopping, decorating, and wrapping gifts. <br />
<br />
If you think you cannot afford holiday care, plan it as part of your gift
giving budget. This will not only be a gift to you, but to your loved one as
well. Relieving tension and stress, as well as enjoying the holidays as the
family member you are and not a caregiver, can bring a memorable and joyful
holiday that will be much more meaningful than a traditional gift. Most
agencies offer gift certificates. Ask family members to purchase care
certificates in place of material gifts.<br />
<br />
The Alzheimer's Store offers a wide variety of products including memory
stimulating activities that can be given as gifts or purchased for
holiday activites.<br />
<br />
<b>8. Understand that holidays’ are frequently a very lonely and depressing
time of year for the elderly.</b> They can get lost in the shuffle, and their
feelings disregarded. Many have lost spouses, and friends. Recent losses are
particularly difficult, including the loss of independence. <br />
<br />
It’s important to take the time to include the elderly in your holiday
preparations and plans.<br />
<br />
Understand and take time to validate their feelings and initiate conversations
about the positive memories they have. Begin a new memory by including a
remembrance of a lost loved one. Whether that is taking flowers to a cemetery,
making an ornament with their loved ones photo, or including a favorite past
time of their loved one in the holiday activities, it will validate their feelings
of loss.<br />
<br />
When it’s a loss of independence, concentrate on engaging your loved one in
simple decision making or tasks that can easily be accomplished. Thank them for
their contributions and add simple compliments. Let them know that you
understand loss of independence is difficult, but you are there for them, and
will help them with what they can no longer do.<br />
<br />
Do not take away independence by doing things for your loved one they can do
for themselves. You want to maintain as much independence as possible for as
long as possible. I always say, if you don’t use it, you lose it. Don’t
contribute to the loss.<br />
<br />
Preparation and these simple tips can make everyone’s holidays something great
and memorable. These tips can apply to most family gatherings and functions.
Keep the list handy for future reference and make all events the best they can
be!</span></span></div>
